At JD3 Mobile DNA Services, we strive to make your testing experience simple, confidential, and professional. Please review our important policies below and complete any required forms following you scheduling your appointment.


Cancellation Policy

  • 24 hours or more notice: Appointments cancelled at least 24 hours in advance are fully refundable or may be applied toward a rescheduled appointment.
  • Less than 24 hours notice: Appointments cancelled within 24 hours of the scheduled time are non‑refundable.


No‑Show Policy

  • If you have prepaid for an appointment and do not show up, the payment is non‑refundable.
  • New charges will apply for any future appointments scheduled after a no‑show.


Changes to Testing

  • Adding tests: Any additions to your testing order will incur extra charges according to our standard fee schedule.
  • Removing tests: If you remove any testing within 24 hours of your appointment, those portions are non‑refundable.


Mobile Testing - Non‑Refundable Once On‑Site

Once our mobile technician arrives at the designated testing location, all fees become 100% non‑refundable, regardless of the outcome of the appointment.

This includes situations where:

  • The client is not present at the scheduled time.
  • Any party refuses to be tested.
  • Any party is uncooperative or unable to provide the required sample.
  • A decision is made to cancel after the technician has arrived without the 24 hour advance notice.

Mobile Testing Policies


📋 Mobile Testing Policy

For your safety and ours, we do NOT travel to personal home addresses for mobile testing appointments.
You may choose a
public location that is convenient and accessible, such as:

  • A commercial business lobby
  • Public library
  • Hotel lobby
  • Coffee shop

Once your appointment is booked, please provide the address of your chosen location. A photo ID is required at the time of testing.


Mobile Testing - Non‑Refundable Once On‑Site

Once our mobile technician arrives at the designated testing location, all fees become 100% non‑refundable, regardless of the outcome of the appointment.

This includes situations where:

  • The client is not present at the scheduled time.
  • Any party refuses to be tested.
  • Any party is uncooperative or unable to provide the required sample.

🚗 Mobile Testing Mileage Fees

JD3 Mobile DNA Services provides mobile testing with the following mileage-based fees:

  • FREE within 20 miles of our testing locations
  • $50 for 20–50 miles
  • $100 for distances over 50 miles

Mileage is calculated using Google Maps based on the address you provide. An invoice for the applicable fee will be sent via email and must be paid in advance to confirm your appointment.

🧬 Required Forms

To ensure timely processing and accurate results, please complete the required form(s) prior to your test. These forms are secure and must be submitted online.

For All DNA Testing:

DNA Testing Questionnaire


This form is required for all DNA test types, including paternity, sibling, grandparent, and legal tests. It gathers information about the parties being tested and helps us prepare your paperwork accurately.


For Immigration DNA Testing:

Immigration DNA Information Form


This form must be completed for all immigration-related DNA cases. It provides us with USCIS case details, embassy information (if needed), and all required documentation.


For Drug Testing:

Drug Testing Request Form


Please use this form to request any drug screening services, including pre-employment, post-accident, or court-ordered testing.

Motor Carriers Medical Examiner's Verification
